Making an Appointment at the Travel Center
We make every effort to accommodate your schedule, but all travel/immunization visits are by appointment only. Appointments are available Monday through Friday.
Travelers should allow plenty of time to complete this process. Proper travel immunizations may require up to 8 weeks in advance if a series of shots is required. Please plan as far in advance as possible.
If you would like to use any of our services, please contact our office at 847.657.5670. A brief travel health history form needs to be completed prior to the appointment (Download the form). The form can be returned by fax 847.657.1759 or email Travel_Center@northshore.org. A separate form is required for each traveler. When we receive your completed form, we will initiate preparation for your visit. Please be sure the forms are complete and signed before returning them - incomplete forms may delay processing.
Since the NorthShore Travel Center is a self pay clinic, payment is required at the time of service and can be made by credit card, cash or check. Self pay travel clinic means we are not equipped to handle any type of insurance correspondence.We do not bill insurance, we do not issue claim forms, and we do not contact carriers for pre-certifications or authorization of any kind. NorthShore University HealthSystem Travel Center is not a Medicare provider. Please keep in mind insurance does not generally reimburse for travel related immunizations or consults.
Outpatient Pharmacy
Conveniently located at Glenbrook Hospital, the pharmacy carries the basic and less usual travel-related medications and insect repellants.
