The Chronic Kidney Disease Clinic, a special program offered by NorthShore University HealthSystem’s Division of Nephrology and Hypertension, provides services to hundreds of patients in the early stages of chronic kidney disease. The clinic, with locations in both Evanston and Glenview, takes a multi-faceted, personalized approach to managing and preventing the progression of kidney disease.
Care in the clinic is facilitated through regular visits with nurse practitioners. In addition to physical exams, the clinic provides control and management of anemia, blood pressure, diet, diabetes, lipids, and mineral metabolism. Education classes are also offered, covering issues such as lifestyle, nutrition, and medication. Advanced education classes explore dialysis and transplant options. The clinic is supervised by nephrologists who have direct communication with the referring primary care physicians.
