NorthShore University HealthSystem maintains patient records in accordance with regulatory standards. These standards do not require that all patient records be permanently retained. Depending on the date and type of service, patient records and films may no longer be available for access and reproduction.

Request a Copy of Your Medical Record

To request a copy of your medical record please complete our medical record request form.

Note: You can now request an electronic copy of your radiology images (MRI, CT, X-Ray, etc). Images will be sent via email.

If you are an insurance, attorney’s office, or record retrieval company, please mail your request to the address below. Faxed or emailed requests will not be accepted.

  • NorthShoreConnect
    If you have an active NorthShoreConnect account, you can request your record through NorthShoreConnect.
    If you are not a member, you may request an access code today.
    The turnaround time to receive records through NorthShoreConnect is 1-3 business days.

    Records requested through NorthShoreConnect can only be released back to the patient.

    To request your medical record be released to NorthShoreConnect, log in, go to "Your Menu" and search "Medical Record Request” Note: radiology images can be sent electronically via email, and Cardiac Imaging cannot be sent via email or to NorthShoreConnect – see below for contact information.

    To view your requested records in NorthShoreConnect, go to “Document Center” then "Requested Records”

  • Request in Person
    To receive copies of your medical records, bring the completed AUTHORIZATION FOR DISCLOSURE OF CONFIDENTIAL INFORMATION form (or complete it at the time of your in-person request). If you send someone else to pick up your medical records, the form must be completed and signed by the patient with your name in the section “I AUTHORIZE NORTHSHORE UNIVERSITY HEALTHSYSTEM TO RELEASE TO” before you come to the hospital. Please bring your driver’s license or photo ID for all in person requestors.

The turnaround time to receive copies of records is 7-10 business days. If needed, patients can come in person to request records at one of our five hospital medical records locations. Our walk-in hours at the hospital locations below.

Computer Readable Electronic Health Information (EHI) Export

This file is a computer readable file (files that a computer or app can read). The files in this export are not designed to be read by humans. Please note that these files can be very large and take your organization a few days to process.

Who can sign to get health information for a patient:

  • The patient (Not the spouse)
  • Power of Attorney if the patient is not able to sign (Legal document must be provided.)
  • Parent (If the patient is younger than age 18 years of age)
  • Parent and minor if the patient is 12 to 17 years of age and is getting psychiatric, alcohol, or drug treatment services.
  • Legal guardian (Proof of guardianship document must be provided.)
  • Representative of the estate for patients who have died. (Copy of the death certificate and a copy of the representative of estate documents must be provided).

Other Types of Requests:

  • To obtain a copy of a birth or death certificate, contact the Illinois Department of Public Health.
  • To check the status of a Social Security card for your baby, contact your local Social Security office.


You have the right to access, review and get copies of your medical record. You have a right to request amendments to your medical record in accordance with our policies. Except where permitted by law, your medical record cannot be released without your consent.
